Economic crisis in Indonesia has brought a decrease on GNP and the utilization of health facilities as well. In order to allocate the  donor\&\#8217;s fund, properly a study on utilization and a real cost of treatment determination has to be done      A cross sectional study on Guidelines development on the assesment of Pharmaceutical cost management and real cost of treatment for in and out\&\#8211;patient at Sidoarjo Furnicipal Hospital on class II and III from pediatric, surgery, internal, comminicable diseases and neurology departmentcia; performed. The data on cost and budget, cost recovery, drug supply and storage, patient access and drug utilization, real treatment cost, real drug cost per curative encounter and patient ability to pay were collected. Data were analyzed descriptively through means and comparison. The study also developed a manual for evaluation of Pharmaceutical cost management       Results of the study as follows :     1. A Guidelines on the assessment of drug cost management and real treatment cost for in- and out-patient at RSU (enclosed)     2. The computation of baseline data for grouped indicators from the year 1998/99 to 2001.     3. The Ability to pay of class III in-patient per family per month is Rp. 14.773,67 or 2,4\% of their earning. The real treatment cost for class III in-patient (Rp. 622.591,00) shows that the cost has tobe paid by the patient just like the treatment cost of out-patient (Rp. 28.133).},
