The objectives of this study were to evaluate the reliability of microscopis diagnosis at different levels of the health care system, between primary health centers and district level and between district and central levels. This study has place in Purworwjo district, Central Java, and was carried out from June to September 2002, in 3 primary health centers (Bagelen II, Bener and Kemiri I). The Strengths of agreement of the diagnosis were expressed by Kappa value. The value of 0.61-1 were accepted as a good agreement of malaria vivax diagnosis among primary health centers and district level microscopist was as follows; Bagelen II produced substantial agreement (Kappa = 0.64 and 0.71), Bener and Kemiri I produced moderate agreement (Kappa = 0.49 and 0.56) and (Kappa = 0.52 and 0.59) respectively, among 2 microscopists from district and central level substantial agreement (Kappa= 0.64 and 0.71) was produced Agreement of malaria falciparum diagnosis among microscopist from Bagelen II, Bener and Kemiri I produced substantial agreement, these were (Kappa =0.63 and 0.71), (Kappa = 0.56 and 0.62), (Kappa =0.60 and 0.66 ) respectively, among 2 microscopists from district and central level there was moderate and substantial agreement, these were (Kappa = 0.59 and 0.62) respectivelyWC 680-950 Tropical and Parasitic Diseases2002Badan Penelitian dan Pengembangan KesehatanArticle
