The quick of growth population and development activity has contributed the continuously decrease of the environmental quality in the city. So that, the environmental quality in the cities and regents as the Agenda 21's priority. These background has effort the government of Indonesia to announce "Healthy City" policy, as the action of community be through healthy life. The study of healthy city has done in 5 (five) municipalities in Indonesia. To know the implementation and the action of the healthy city policy in five municipalities in Indonesia. The study has been done in 5 (five) municipalities in Indonesia such as Medan, Semarang, Denpasar, Manado and Balikpapan, Indonesia with qualitative approach to describe government policy on Healthy Cities in selected municipalities. Interview technique had been undertaken by in depth interview and focus group discussion toward Municipality Officer, Municipality Development and Plan Board, Health City Forum, Municipality Areas Development and Houses Office, Environmental Impact Control Board, Tourism and other linked sectors. From 5 (five) selected area, 4 (four) cities had already undertaken Healthy City Policy, while Balikpapan Municipality has done it in the longest period of time. On the other hand, Semarang Municipality have had not the Healthy City Forum and still in coordination with related sectors as well. In general, problems in undertaking of Healthy City were socialization and coordination.
