Lokasi penelitian di tiga kecamatan (Porong, Gedangan, Tarik) Kabupaten Sidoarjo Jawa Timur. Hasil penelitian menunjukkan bahwa hampir semua sektor terkait berperan serta dalam memberikan masukan dan proses pelaksanaan program UKS. Untuk luaran ternyata pengetahuan, sikap dan perilaku murid-murid sekolah di SD UKS kategori II dan III menunjukkan adanya perbedaan yang bermakna pada tingkat signifikasi=5% dengan murid-murid dari SD UKS kategori I. Demikian pula dengan angka rata-rata absen karena sakit.
