Endemisitas dan densitas mikrofilaria yang rendah membutuhkan pemeriksaan yang tidak tergantung periodisitas, spesifik, sensitif dan terdeteksi pada masa prepaten, inkubasi maupun infeksi tunggal cacing dewasa. Excretory-Secretory antigen (E-S Ag) beredar dalam peredaran darah (circulating antigen), ditemukan pada infeksi paten dan menghilang pasca pengobatan. Deteksi dilakukan dengan MoAb pada ELISA. Antibodi monoklonal ES34 telah digunakan untuk deteksi W.bancrofti di Cina dan dilaporkan memberikan hasil 80% spesifik. HC11 dipakai untuk B.malayi. Trop Bio juga telah mengembangkan kit yang spesifik dan sensitif untuk deteksi W.bancrofti. Deteksi antigen menggunakan MoAb HC11 dan deteksi IgG4 menunjukkan infeksi filariasis. Telah dikumpulkan MoAb untuk pemantapan ELISA dengan ES34, HC11, penggunaan kit dengan Og304, deteksi IgG4 dan pengumpulan 360 serum dari daerah Riau, dengan 27 penderita elefantiasis. Penggunaan HC11 belum berhasil sedangkan ES34 menunjukkan reaksi non spesifik yang tinggi. Penggunaan Og4C3 menunjukkan sensitivitas 40% pada mikrofilaremia 0-10 mf/20 ul dan 100% pada >10 mf/20 ul. Sensitivitas deteksi IgG4 pada B.malayi adalah 81,8% sedangkan spesifisitasnya 50%.
