Sebagai responden dipilih semua petugas kesehatan lingkungan di semua Puskesmas Kabupaten Gresik. Tujuan dari penelit ian ini adalah mengevaluasi pencatatan dan pelaporan kesehatan lingkungan Puskesmas terutama mengenai : kelengkapan dan kebenaran pengisian, kebenaran mendapatkan data dan apakah data tersebut sudah dimanfaatkan untuk monitoring. Evaluasi dilakukan dengan berpegang pada Buku Pedoman Pembinaan Pencatatan dan Pelaporan Kanwil Departemen Kesehatan Republik Indonesia. Dari hasil penelitian dapat disimpulkan bahwa para petugas kesehatan lingkungan Puskesmas Gresik masih belum seluruhnya mengisi Form Pencatatan dan Pelaporan secara lengkap dan benar. Tentang kebenaran data masih juga diperlukan penanganan yang serius yaitu adanya kerjasama antara petugas kesehatan lingkungan Puskesmas dan Pamong desa serta dokter Puskesmas agar data mendekati kebenaran dan dapat dimanfaatkan untuk monitoring.WY 150-170 Nursing in Special Fields of Medicine1990Puslitbang Pelayanan Kesehatan SurabayaMonograph
